The SA Health and Dental Plan… It’s Nice to be Covered.

Coverage is only available to Full-Time students.
Full-time students are automatically enrolled. Fees are assessed on your Tuition Fee Schedule.

Coverage is valid 24 hours a day, 7 days a week from September 1, to August 31 the following year – 1 complete year.

The Students’ Association of Medicine Hat College Health & Dental Plans are managed through the Campus Trust. For more information pertaining to the Trust please contact the Student Benefits Administrator in the Students’ Association office.

To check to see if a particular prescribed drug is covered please check:

Choose the SAMHC Plan and then Click on Drug Coverage Search. There you can enter your DIN# to see if it is covered under the Student Drug Formulary.  If your drug isn’t covered you can contact our office to get a Drug Exception Form. This form will need to be signed by your Doctor and then submitted to the Campus Trust for review and/or approval.

Opt-Out Process
If you have comparable coverage under an alternate plan, you may opt-out. You must be able to provide proof of alternate coverage (see sample) by detailing the insurance provider and policy number. OPT-OUTS MUST BE DONE AT THE STUDENTS’ ASSOCIATION OFFICE or Online at www.sbta.ca

Fall Registration: September 13/2010
Winter Registration: January 17/2011

Add Family Coverage

You now have the option of completing your couple/family enrolment form online at www.sbta.ca
Dependent coverage is available for your spouse and/or all unmarried dependent children, step-children, or adopted children under the age of 21 or such children under the age of 25 who are full-time students at a College, University or accredited institute. Forms are available at the Students’ Association office and must be submitted to the SA by:

September 24/2010** Fall Registration
January 28/2011** Winter Registration

It is the students responsibility to inform the SA that Family Coverage is desired; exceptions will not be made after deadline dates. **Please note that in subsequent years the family enrollment deadlines will be the same as the opt out deadlines.

Pay Direct Card
Your student health plan provides a pay direct service for prescription drug claims. This service should be available within 2 months of the start of the semester. This delay is due to the loading of the SAMHC student information to the pay direct system. If you are affected by this delay, please use the manual claim forms reimbursement method.

Once you have received your pay-direct card, you will present the card to your pharmacist and your prescription. You will then be expected to pay the remaining 30% co-insurance as well as any amount of the dispensing fee that exceeds $5.00.

Remember that you can always download a Pay-Direct Card above if you have not yet picked yours up. Remember to take your student ID card along to the pharmacy.

Please note: some pharmacies do not participate in electronic processing of claims.

If you experience any problems, please contact the SAMHC at 529-3925.

Submitting Manual Claims
For other medical claims (and dental claims) you will need to complete a manual claim form. These can be picked up from the SAMHC Office or downloaded from this site (see above). Complete all pertinent information and mail to the address noted on the claims form. You should expect reimbursement within 2 weeks

Claims should be submitted within 6 months of date of receipt. Your cooperation with this is greatly appreciated.
