The SA Health and Dental Plan… It’s Nice to be Covered. Coverage is only available to Full-Time students. Coverage is valid 24 hours a day, 7 days a week from September 1, to August 31 the following year – 1 complete year. The Students’ Association of Medicine Hat College Health & Dental Plans are managed through the Campus Trust. To check to see if a particular prescribed drug is covered please check: Choose the SAMHC Plan and then Click on Drug Coverage Search. There you can enter your DIN# to see if it is covered under the Student Drug Formulary. If your drug isn’t covered you can contact our office to get a Drug Exception Form. It is the students responsibility to inform the SA that Family Coverage is desired; exceptions will not be made after deadline dates. **Please note that in subsequent years the family enrollment deadlines will be the same as the opt out deadlines. Useful Links Alberta Health & Wellness Pay Direct Card Once you have received your pay-direct card, you will present the card to your pharmacist and your prescription. You will then be expected to pay the remaining 30% co-insurance as well as any amount of the dispensing fee that exceeds $5.00. Remember that you can always download a Pay-Direct Card above if you have not yet picked yours up. Remember to take your student ID card along to the pharmacy. Please note: some pharmacies do not participate in electronic processing of claims. If you experience any problems, please contact the SAMHC at 529-3925. Submitting Manual Claims Claims should be submitted within 6 months of date of receipt. Your cooperation with this is greatly appreciated. |
